7th Grade Health Requirements

  • Please review this important information about mandatory health requirements for your soon to be 7th grade student.  If you have any questions about these health forms, please email Tina Tarvin.

    The State of Florida laws require that students entering 7th grade must have both of the following items turned in to our Front Office BEFORE starting school.  

    1. A School Physical Examination (by doctor/healthcare provider) on Form 3040 is required and must be dated on or after August 12, 2023.
      * Part 1 of the Physical Form - Must be completed and signed by the parent/guardian and Part 2 - Must be completed and signed by the doctor/healthcare provider.
    2. Immunization Form DH680 with the following:
      • Tetanus/Diphtheria/Pertussis (Tdap)
      • Varicella series (chicken pox) vaccine OR documentation of history of Varicella Disease
      • The Hepatitis B Vaccine series
      • MMR Vaccine series (measles, mumps and rubella)
      • Polio vaccine series
      • Diphtheria-tetanus-pertussis vaccine series

    Incomplete Form DH680:  If an incomplete Form DH680 is turned in to the school, it is invalid and must be returned to the parent/guardian to obtain healthcare provider/doctor’s corrections.

    Please don’t delay in turning in these documents; medical appointments fill up quickly in summer.
    Once forms are completed, please send to our school as soon as possible.

    How can you turn in these forms to our school?

    1. Mail the forms to Jupiter Middle School - 15245 N. Military Trail, Jupiter, FL. 33458
    2. Scan and email to Tina Tarvin.
    3. Drop off the forms. (Front Office hours in summer are Monday - Thursday, 8am - 4pm.)
