    How to instructions:

    • Either register for a new account or sign in to your account.
    • "Order" the course for Florida (they are FREE)
    • Complete each course and answer the questions
    • Download your certificates of completion for each, print and attach them to your athletic packet.

Athletic Eligibility Packet

  • MPORTANT NOTES REGARDING ATHLETIC ELIGIBILITY PACKETS

    All forms are available on the District Athletics page.

    A new packet must be completed each school year in its entirety prior to participating in ANY sport. This includes off-season conditioning, preseason conditioning, tryouts and any other workout.

    NOTE: If you have completed and submitted an Athletic Eligibility Packet in the current school year, you do not need to resubmit. One completed packet is all you need per school year.

    You MUST use the documents supplied in the packet. No alternative documents may be used. Doctor's notes cannot substitute for page 3 of the physical (FHSAA EL2). Page 3 of the physical is completed only if the student athlete is referred to another physician for clearance.

    We CANNOT Accept The Following Documents In Place Of The FHSAA EL2 Sports Physical

    * State Of Florida School Entry Health Exam
    * Florida Certificate Of Immunization
    * FL Department Of Health Medical Evaluation

    Forging or falsifying ANY information within the document will result in the suspension of the student athlete 365 days from the date of discovery (FHSAA Handbook 9.1.2.2).

Required Athletic Try-Out/Insurance Fee

  • All high school interscholastic athletes will be required to contribute $75.00 toward the cost of interscholastic athletics accident insurance. This school year contribution will be used to help offset the School District's cost in providing quality accident insurance for our athletes. Coverage may begin AFTER your primary insurance coverage processes a claim. Athletes MUST use their Primary Insurance Network first, before using the school insurance.     Option
     1: A student-athlete may choose to pay a ONE-TIME $75.00 fee.

    Option 2: A $10.00 non-refundable fee can be submitted to try-out for any sports team. Upon making the team, the student-athlete is responsible for remitting the balance of $65.00 before participating in further practice or games/events. Until the $75.00 annual limit is reached, the student-athlete must remit an additional $10.00 try-out fee for each sport.  

    How to pay: Student Cash Online via debit or credit card or we will accept a check or money order made out to Park Vista Community High School. Cash is NOT accepted. Payment is non-refundable.
