To support families and schools with a contactless sign-out process, the Middle School After-school Program will be requesting Parent/Guardian to submit an electronic signature for student pick-up from the MSAS Programs.

Parent/Guardian will:

• Call the after-school director or main office upon arrival to the school.
• Complete and submit the MSAS After-school Student Sign Out Form for each student being picked up daily.
• The form must be submitted from the designated pick-up location only.
