Athletic Eligibility Packet and Waivers

In order for a student to participate in high school athletics, they must have:
- A minimum cumulative grade point average (GPA) of 2.2
- Be clear of any school obligations/financial fees
- Have earned a minimum number of course credits based on grade level:
- Start of 10th grade: 7 credits
- Start of 11th grade: 13 credits
- Start of 12th grade: 19 credits
- Have a minimum number of Community Service Hours (visible on SIS) based on grade level by October 12th:
- 10th grade: 5 hours
- 11th grade: 10 hours
- 12th grade: 20 hours
- Submit the State/District-required eligibility packet online via Aktivate.com (which includes attaching NFHSlearn.com course completion certificates)
- Submit the Athletic Insurance fee payment (may be paid on School Cash Online).
Note: some Dwyer athletic teams may have additional packets/contracts required for participation.
How to Submit the Required Documents, Courses, & Fees for Athletic Participation:
Online via www.Aktivate.com:
- Log in, or Create Account if this is your first time using Aktivate.com
- Complete & upload ALL required documents to your Aktivate.com account.
- Check your Aktivate.com account for "Cleared" status
- If not cleared, complete and resubmit the missing/incomplete/incorrect document(s).
- May be submitted online any time, any day of the week.

The following documents will need to be completed and uploaded to your Aktivate.com account:
- Summer Participation Waiver (only required for participation in Summer workouts)
- PBCSD Athletic Eligibility for Students Form
- PBCSD Medical Consent Form
- PBCSD Interscholastic Athletics Accident Insurance Form
- FHSAA EL2 Participation Physical (any other Physical forms NOT ACCEPTED)
- FHSAA EL3 Consent & Release
- NFHS Concussion Course Certificate
- NFHS Heat Illness Course Certificate
- NFHS Sudden Cardiac Course Certificate
- Birth Certificate (for proof of age purposes only)
- Receipt of Athletics Insurance Fee from School Cash Online (payment option for 2024-2025 school year will open July 5th):
- NO CASH ACCEPTED!
- $75 participation Insurance Fee
