Eligibility Packets

Attention student-athletes and parents/guardians:

Per Florida High School Athletic Association (FHSSA) and Palm Beach County School District policy, completion of all of the following forms and online courses are required in order to be eligible to participate in athletics at PBGHS:

School District of Palm Beach County:

1. Athletic Eligibility for High School Students (form 1588)
2. Student Medical Consent for Athletics (form 1589)
3. Interscholastic Athletics Accident Insurance (form 2608) (Pay for insurance at SchoolCash Online)

FHSAA:

1. EL2- Sports Physical
2. EL 3- Consent and Release from Liability
3. Transfer and Home-Schooled students will be required to submit additional FHSAA forms.

Online Courses:

1. Concussion for Students
2. Heat Acclimation and Heat Illness Prevention
3. Sudden Cardiac Arrest
